Nottinghamshire County Council has launched a new webpage explaining its Transitions Pathway for Young People aged 13-25 with disabilities.

The attractive interactive “road” breaks down how support is given across education, health and social care and also includes milestones for when meetings should take place and reviews of your situation should be made.

What’s more the page also features speech, reading and translation support, a link to a video which explains the pathway a little better and a glossary of terms so you can better understand the help that’s available.

Council Market Development Officer Phil Smith said: ‘Transitions to adulthood can be a time of great anxiety for young people with special educational needs and disabilities and their families.

“This interactive transitions pathway provides much needed information, advice and guidance on the transitions process in terms of what should be happening and when it should be happening and signposts to the support and services that are available to  young people and their family.’
